This template should be used to apply for a special condition to be imposed on an accreditation to allow the creation of ESCs of 2019 vintage where a reasonable assurance opinion from an auditor cannot be obtained for reasons relating to the COVID-19 pandemic, that is COVID-19 affected ESCs.
